1 Assessment coordinator Request / update on complaints/ practices
2 Team Leader Request update on complaints/malpractices if no information is available from level 1
3 Quality Assurance Manager Escalate if No information is available from level 1/Team leader or if the information received does not meet expectation.
4 Executive Director Escalate if not resolved at above levels for further process with reveiws
5 CEO & Director Final level of escalation when no satisfactory information is available from levels of escalation
